Records the visit
With the patient’s consent, from the laptop already in the room. No phone in a pocket, no separate device to remember, no hardware to buy.
Ambient documentation for gynecology
SimaNotes listens to the visit you are already having and drafts the note from it — structured the way you write, with the coding evidence attached. You read it, change what you want, and sign it in your EHR.
SimaNotes drafts. The clinician reviews, edits, and signs.
It does not diagnose, it does not decide, and it does not sign anything.
A gynecology visit produces a history, an exam, an assessment, a plan, orders, and a billing decision that is rarely obvious. Most of that is settled in the room. Almost none of it is written down there.
What gets written down happens later — after clinic, from memory, when the details that would have justified the code are the ones that faded first. Ambient documentation is not about typing faster. It is about writing the note while the visit is still the source.
What it does
With the patient’s consent, from the laptop already in the room. No phone in a pocket, no separate device to remember, no hardware to buy.
A structured draft in your own voice and format — HPI by topic, assessment and plan with sub-bulleted actions, the recaps you actually dictate. Section by section, so you take across what you accept.
The preventive-versus-problem call, made explicitly, with the evidence for each code quoted from the visit — and the work that was considered and deliberately not billed separately.
Getting a fluent paragraph out of a transcript is close to a solved problem, and it is not what makes documentation expensive. The expensive part is the judgment on top of it: whether a well-woman visit that turned into a problem discussion is one service or two, whether a modifier is defensible, and whether the note as written would survive somebody reading it back a year later.
SimaNotes is built by someone who works denials and coding for a living, which is why the draft argues its billing call rather than asserting it — and why it will tell you when the safer answer is not to bill separately at all.
What it is not
Nothing SimaNotes produces enters a chart without a clinician putting it there. Every section is a proposal with a Copy button next to it — you paste it in yourself — and everything it suggests about coding is an observation for you and your biller, not a recommendation, and never a claim.
The audio exists to become a transcript, and once the note is finalized it has no reader. It is destroyed at finalization, and in no case held longer than 24 hours. What we keep, and for how long →
SimaNotes is early and working with a small number of practices by design. If gynecology documentation and the coding underneath it is a problem you recognise, we should talk.
